(1) The original expression used for "over-wound" is an analogy meaning something along the lines of "bringing something back to its correct shape (working as intended)", essentially by "winding the springs of its mechanism so that it can work properly again". In the case of Baroque, where the world is distorted beyond recognition, we can see that this is meant to be ironic: by "adjusting the spring", the world has become barren and grim, instead of becoming something closer to the Eden that we would expect. But there is yet another layer to this allegory once we consider the ending, which makes it a really clever piece of wordplay.
Ultimately, I opted for "over-wound" because winding a spring too much breaks it, which conveys the nuance that, despite our efforts to "wind the world" so that it would work again, it is "not working as we intended". Additionally, "wound" might also evoke the idea of damage or harm (being hurt), which, while not present in the original, I think is fitting regardless, due to the nature of the game.

(2) I considered using "dysfunctional people" instead, but this would change the meaning considerably. The nuance would shift from "people with an impairment" that prevents them from "functioning" correctly, to "people who are not performing properly" (not necessarily due to an impairment, mental or otherwise). The only downside is that "disabled people" sounds slightly unnatural here, and perhaps even more offensive than in japanese (although the original term wouldn't be used lightly in that languge, either). While it is usually not used this way in English, I prefered to maintain the original meaning.
